Referral procedures

Referral Criteria

Cam-mind offers friendship and support to people with mental health problems living in the community by providing day services and a befriending scheme. The minimum age for our services is 18; applicants must be able to interact socially and have a medical diagnosis for the condition of their mental health.

Referral Form

All potential users of our day services and befriending scheme should complete a referral form in collaboration with a referrer.

The form contain two parts:
Part 1 to be completed by the client, with help from the referrer
Part 2 to be completed by the referrer, in collaboration with the client

and our address to return the form to:

An acknowledgement letter will be sent to both the client and referrer
on receipt of a referral form by the Cam-mind office.
